Why-why analysis
Why – Why analysis is the simple tool used by the observer to identify the causes of the problem, incident or breakdown at the workplace.
The tool used as the general internal questioning technique for getting beyond symptoms and uncovering root causes. it is most common application to identify root cause analysis is completed as part of incident investigations and quality improvement activities. A many times, the root cause of the problem can be identified by asking Why – why at least 5 times and the repetition of such problems can be prevented by taking countermeasure immediately.

Suppose you have manufacturing facility having issue with machine breakdown physical phenomenon “low air pressure” identified at the machine during in the process. Generally, low air pressure is related to ball valve as every manufacturing maintenance peoples as well management knows that it’s easier to track the cause, and takes final actions on open ball valve. valve was closed
Why valve was closed?
it was in passageway
why valve in passage?
It was at wrong location
Action: The open ball valve was on wrong location, the corrective action will be immediate countermeasure and relocate the value at correct location.
Now days are gone to repair machinery, equipment and forget about the breakdowns and real problems, when second time breakdown happen for that machinery or equipment same are proceed.
