In manufacturing unit,  product quality, product price and others important thinks are depends on the process / procedures / methods for the manufacturing product, if the process or procedures, method has nay defects that directly effect on product quality and its price, hence number of customer can be reduce due this important part of the transition of material sale forecast.

Employee / Workmen are directly concern with product manufacturing hence the detailed information of the process & method are used can be better understand, which procedures and method is useless or effective and its concept are much better can understand due to routine experience. For the amendment in process / procedure or method is necessary if the losses can cause due to it, when employee / workmen fill that think can suggested to management for the change the right procedure or method for the improvement in quality or costing reduction. For the this formal records suggestion form is used to recording the information & comparison of both “ current procedure / method against suggested procedure / method” on base of the testing or other parameter, inspection, trials which procedure / method is effective for the product are decided by management

For this exercise, required fields for the suggestion form which is basic fields to record information collect from suggestion team as below:

1. Suggestion number and date
2. Suggestion for Department and Suggestion for Process / Procedure / Method details
3. Brief description of suggested method / Process – when individual employee / team of employee are suggest any method / process or procedure for used instead of current, details of brief description are need to compare against it.
4. Present method / Procedure – what current method / process or procedure are used in the particular department which against suggestions is received.
5. Disadvantage of present method / procedure
6. Suggested Method / Procedure
7. Advantage of suggested method / Procedure / Process
8. Drawing / Sketch / Process flow / Diagram etc.. for justification
9. Suggestion team member list
