Amendment / supplement request document is established to application for amend or supplement document used internally.

 

Amendment / supplement request document is prepared & apply in the requirements for the requesting to amendment or supplement document as per internal system requirements, or as per department internal used, the document is needs to amend or supplement on department internal used when any employee having a trouble or needs to change the document as per internal system, procedures or process requirements are conducted for the amendment or supplement, concern employee is discuss with department head for change required in the document to meet the internal requirements that department head is sending request letter to management representative to amend / supplement document as per system requirements. When the management representative received application for the amendment / supplement request from department, that management representative and concern team is reviews the records and documents for the approval and further action for amendment or supplement document as required.

 

Amendment / supplement request document is prepared by department head and sending to management representative to recommending amend, change or enhancement in document as requirements of current system in department which is needs to meet processes are established and managing in workplaces, and the management representative is amend or supplement document accordingly after verification / reviews of the processes and format is provided for system documentation. Amendment / supplement request format is used for requesting document / format amendment, change, enhancement that is supplement as per internal departmental processes to meet the requirements of documentation, the request send by department head to management representative to amend or supplement master list index and master formatting system that handled by management representative as per standards system.
