Equipment incident report is defined as a document that describes the information of the incident of equipment damage, loss or theft. This report will help to identify root cause of incident for eliminate feasibility of risk in workplace.   8. Recent Service Date
  9. Service by & Sign. – Name of the person who recently service task performed.
  10. Manager maintenance signature – To acknowledge the incident, here after signature is required in this regard.
  11. Location at used – Name of the location where equipment is installed and used.
  12. Incident reported by – Name of the person who reported for equipment incident, on base of the information this report is generating.
  13. Equipment Operator name
  14. Incident Description
  15. Person Injured / Environmental effect? – This is very important and necessary information in this form. In incident, any personnel injury or environmental effects are recorded that, the description of incident related to injury and or adverse effects on environment are need mentioned in detailed.
